How to Apply for a Medical Card

Step 1

Physician Certification of Diagnosis: This can be difficult or even uncomfortable but you should ask your physician if cannabis is an option for you. A large number of Illinois Physicians cannot or will not certify patients for medical cannabis  yet. If this is an issue you are familiar with please contact us right away. We can help you procure a medical marijuana card.

Step 2

Access the Illinois Cannabis Tracking System online at https://icts.illinois.gov
NOTE: These steps are complicated and can be completed at our clinic with FREE assistance.

(IT IS AGAINST THE LAW FOR ANYONE TO CHARGE YOU A FEE FOR REGISTRATION ASSISTANCE FOR YOUR CANNABIS CARD)

Step 3

Create an account and click MCPP Patient Registration to begin a new patient application and upload all supporting documents. (Physician Certification, ID copy, 2 proofs of residency, photo for ID, DD 214 and disability statement of benefits if applicable)

Print & Shop

Print your provisional access card. Head over to you dispensary you chose during the application process and begin healing with cannabis. (dont forget your ID & provisional access letter or you won’t get in)
